Form Tips

  • Some forms are in an electronic system that will require you to log in with your Net ID while others are a PDF.
  • Input your actual Faculty Advisor on all forms, except a Leave of Absence (put kstucki for LOA)

Course, Registration, or Requirements

Courses, Registration, or Requirements
  • LDL Student Online Program Plan: Create this as a new student and maintain it throughout your degree program
  • Late Add or Drop: This form is completed through the Graduate Student Forms process.
  • Request for a Waiver of a Requirement: This form needs to be approved by your advisor, the department, and the College
    • For EdM, the 500-level requirements can not be waived (no exceptions)
    • For doctoral students wanting to switch to the EdM, if you have already earned 32 credits to apply towards the EdM, it is common, but not guaranteed that the Social Foundations requirement can be waived (typically all other EdM requirements are fulfilled through the EdD requirements)
    • If you have taken a course that you believe fulfills a requirement, but you are told prior to graduation that it does not count. First check in with Dr. Francis to discuss your situation, then she’ll advise if this is the next step. You may need a petition to have the course count towards your degree. Please note that the Audit Tool is not correct, so please ensure that you rely on our requirements page and also consult with Dr. Francis if you have any concerns.
  • Independent Study (EPOL 595) Contract: You must first speak with Dr. Francis before filling out an IS contract. Each IS course can be for 1 to 4 credits. You are allowed up to a total of 12 IS credits towards your doctoral degree to be used as a regular course elective or your Early Research Project (PhD students only). Input your faculty advisor as the instructor. Best practice is to choose full semester for the duration.
  • In Absentia (on-campus coded students only who are registered, but over 50 miles away from campus): https://go.grad.illinois.edu/student-portal. Please note that you must be registered for courses prior to submitting the form.

UIUC and Transfer Credits

UIUC Credits
  • Apply course credit towards your degree that you earned prior to starting your degree program: If you began your coursework prior to officially beginning your program, such as a non-degree seeking student, an un-earned UIUC Masters degree, a Graduate Certificate, or the Instructional Design MasterTrack Certificate through Coursera, fill out a Graduate Student Petition to have that coursework applied towards your degree.
  • Informational: EPOL 599 courses will show as deferred until after you graduate, so there is nothing to worry about there
Transfer Credits

Transfer credit from other institutions or courses completed towards another earned UIUC degree are not applicable. Note that transfer credit from other institutions or courses completed towards another earned UIUC degree are not applicable towards the 64 credits needed for the doctoral degree.

Lapse in Enrollment or Withdrawal

  • Leave of Absence (LOA): If you do not register in Spring or Fall, fill out a Leave of Absence. It is not necessary during Summer. After two total semesters (consecutive or non consecutive) of LOA, you may not be able to have access to University resources, such as the library or your email address. Note that during your leave of absence you may also lose access to certain resources.If you need to take more that two semesters of LOA, you will need to fill out a Graduate Student Petition form
  • Withdraw from courses for the entire term: University of Illinois Course Withdrawal/Cancellation Form
    • While you can withdraw from a course for a given term if you are enrolled for the other term, you are not able to fully un-register for courses on your own if it will leave you with zero enrollment, even if it is before the drop deadline
    • You must also fill out a Graduate Student Petition form

If you have not registered for awhile, you may need to petition for re-entry. Start the petition process early.

How do I know if I need to Petition for Re-Entry?

If you don’t currently have access to register for the current term, this may mean that you need to petition for re-entry.

You may also need to complete the annual evaluation from most recent January in order to register (if the registration error message says advising hold – that means you did not submit the evaluation by the deadline and have not contacted Grad Services to have the hold lifted).

  1. If you have any incompletes, you will want to finish those before petitioning for re-entry. Contact Dr. Francis to discuss your re-entry plan.
  2. Go to https://choose.illinois.edu/apply/?sr=be71fa9f-8b37-44bf-9029-121abed2596c and select “Start New Application“, then select “Graduate Student Petition” from the drop-down menu (near the bottom).
  3. This petition goes through at least three or four people, so please allow time for the approval process to occur.
  4. Receive confirmation that you have been approved for re-entry
  5. Complete the Annual Evaluation (look for an email sent your Illinois email address around in January of the most recent year or email grad services to ask for the link)
  6. Once you finish the evaluation, then you need to email gradservices@education.illinois.edu to request the hold be removed.

Program, Degree, or Advisor Change

Program, Degree, or Advisor Change
  • On-Campus vs. Online: If you need to switch from the online to on-campus program or vice versa, fill out the Graduate Student Petition form and indicate a change of curriculum. Note that classes are still all online except for some PhD requirements.
  • Change of Advisor: It is possible that during your degree you decide to switch to a different concentration or advisor. Each requires its own form. To request a change in concentration, fill out the Graduate Student Petition form and indicate a change of curriculum. Be sure to discuss your plans with your current advisor before filling out the form, as both the outgoing and incoming advisor and program leader will need to sign both forms.
  • Changing for Doctoral Degree to Masters Degree: If you are enrolled in the Doctoral program, but decide to stop at the Master’s, you’ll need to fill out a Graduate Student Petition form and indicate a change of curriculum. Depending on the courses you have taken, you may need to submit a separate request to substitute a Master’s requirement that you have not completed. Wait to submit the second petition (for course substitutions) until after your degree change petition has been approved.

  • PhD Only! Research Area Requirement Plan: Fill this out prior to starting your Early Research Project
  • PhD Only! Fill out an Independent Study Contract (see registration forms above) if you want to earn credit for your ERP (note that these credits would not replace any for-credit requirements)
  • PhD Only! Early Research Project Request: Fill this out once you are ready to schedule your ERP. Coordinate with Dissertation Advisor prior to submitting the form

Registration for the EDS seminars and requests for examination all require a form.  Refer to the list below on which forms are needed and when, but most of the actual form links can be found on the Graduate Student Forms page, including the Independent Study contract, for situations where this registration option is needed. Prior to beginning the EDS, please create your EDS Plan and Progress Report form.
